Meal Plan Scholarship Disbursement REQUIREMENTS DISCLAIMER

• Only students that have an eligible scholarship(s) are able to submit this form. The eligible scholarship(s) must be able to be applied towards meal plan or flex charges.
• Please note that any Academic Merit Scholarships or Tuition-Based Scholarships are NOT eligible to go towards a meal plan or flex dollars.

** IF THE ABOVE STATEMENTS ARE TRUE YOU CAN PROCEED WITH THE REQUEST **

If you are already receiving a meal specific scholarship, you do not need to submit this form. Your scholarship will automatically pay towards your meal plan. Example: If you are a JSU Honors Scholarship recipient and are receiving the JSU Honors Meal Scholarship, you do not need to submit this form. Instead you will need to let us know which plan you choose by filling out the Meal Plan Selection Form. You can notate in the comments that you are receiving an Honors Scholarship, Athletic Scholarship, etc.

This voucher is to be used if you have an unrestricted scholarship available on your MyJSU account and wish to use those funds to purchase a meal plan or flex dollars. The scholarship must be currently posted to your account with all other school fees already charged and paid. If these statements are true, you can proceed with the voucher process.
